2 Quick Apply Full-time 9 hours ago Full Job Description MDS Coordinators responsible for Care Area Assessments and care plans, in accordance with current federal and state regulations and guidelines.
You must have your LPN license or RN license for this postition.
Acts as in-house Case Manager demonstrating knowledge of residents health status, critical thinking skills to develop care plan to help residents achieve the highest level of functioning we can.
Work hand & hand with the director of nursing to review recent admits, discharges and quality of care.
Utilizes critical thinking skills and collaborates with therapy staff to select the correct reason for assessment and Assessment References.
We are currently looking for a new MDS Coordinator.
We are a Family owned and ran Skilled Nursing Facility that has been in business over 40 years.
We as a facility take a lot of pride in the care given by our staff and in return try to go above and beyond for our employees.

Completes admission and discharge assessments and maintains tracking system for admission/re-entry/discharge including timely completion of certifications and re-certifications for skilled coverage.
$36-$40 an hour starting out and will negotiate wage Electronic charting system
